Personal Assistance
A PA provides direct assistance with all activities of daily living, including grooming, hygiene, meal preparation, budget management, attending appointments and building social skills.
Supported Living
SL provides individualized service and support enabling individuals to acquire, retain, or improve skills necessary to reside successfully in a family environment of trained caregivers.
Respite
Respite care is a service provided to an individual when unpaid caregivers are incapacitated or absent due to health, hospitalization, injury, or simply the need for some relief from care giving duties.
Supported Employment
SE is individualized support designed to enable individuals to acquire lifestyle supporting employment within the community.
Community Based Day
CB Day enables individuals to participate in meaningful and productive activities in integrated settings with other community members who may or may not have disabilities.
Transportation
Transportation is provided to and from approved activities as specified in the plan of care.
